Why Waist Circumference Predicts Health Better Than BMI

A study published in PLOS ONE in 2011 followed 225,712 U.S. women and men for up to a decade, recording 20,977 deaths along the way. The number BMI doesn't see

BMI is a ratio of weight to height. It cannot tell the difference between muscle, bone, and fat, and it says nothing about where fat sits on the body. The Leitzmann et al. study tested whether waist circumference — a rough proxy for abdominal fat — added information that BMI didn't already capture.

It did. After adjusting for BMI, age, sex, race, smoking, and alcohol use, people in the highest waist-circumference category had a 1.37-fold higher risk of dying from any cancer and a 1.82-fold higher risk of dying from cardiovascular disease, compared with those in the lowest category. These effects held even among people with similar BMI, which is the key finding: waist size carried risk information that body weight alone did not.

Where the gap is starkest: lung and respiratory disease

The clearest illustration of BMI's blind spot came from lung cancer and chronic respiratory disease. Comparing the highest to the lowest waist-circumference groups, the study found a 1.77-fold increased risk of lung cancer death and a 2.77-fold increased risk of death from chronic respiratory disease.

BMI, measured the same way in the same people, showed no meaningful signal for either outcome — relative risks of 0.94 and 1.18, statistically indistinguishable from no effect. In other words, two people with identical BMI can carry very different risk profiles depending on where their fat is distributed, and waist circumference picked up on that difference when BMI could not.

What this study can and can't tell us

This is a large, well-documented cohort, but it has real limits. Participants were mostly older, mostly white members of the AARP cohort, and both waist and weight were measured once, at the start of the study. That design shows a strong statistical association between waist size and later mortality, not proof that a bigger waist directly causes disease. Younger or more diverse populations may show different patterns.

The findings also don't replace clinical judgment. A single measurement, at any point in time, is a snapshot. What it does argue for is paying attention to waist circumference as a metric worth tracking over time, alongside — not instead of — a broader picture of health that any consumer tool should be honest about being an estimate.
