When people start creatine, the first question is usually about the scale. What the researchers tested

The study followed thirty moderately active women through a standard creatine loading protocol: 4 doses of 5 grams a day for 5 days, compared against a placebo group. Because fluid balance can shift with the menstrual cycle, the researchers tested everyone across both the follicular and luteal phases, using bioelectrical impedance spectroscopy to measure total body water, extracellular fluid, and intracellular fluid.

That design matters. It let the researchers separate water changes caused by creatine from water changes that might just be part of a normal cycle. The comparison is specific to this protocol and this population, and the authors are upfront that a 5-day loading window in 30 women does not automatically extend to men, to longer-term use at maintenance doses, or to much larger groups.

The water moved, the scale didn't

During the luteal phase, the creatine group showed clear increases: total body water rose by 0.83 liters compared with a 0.62-liter drop in the placebo group (p = 0.021). Extracellular fluid rose 0.46 liters versus a 0.19-liter drop (p = 0.013). Intracellular fluid rose 0.74 liters versus almost no change in the placebo group (p = 0.041). Every fluid compartment moved in the same direction.

And yet body mass itself didn't differ meaningfully between groups (a difference of 0.40 ± 0.50 kg, p = 0.427). The researchers describe this as creatine acting as an osmotically active molecule, pulling water into muscle cells on its own terms, separate from whatever the menstrual cycle happens to be doing with fluid at the same time.

Why 'creatine bloat' isn't fat
